A multidisciplinary doctor / nurse / caregiver team (possibly accompanied by a physiotherapist, dietitian, psychologist, etc.) will travel daily to ensure the continuity of care and will be in direct and permanent contact with your prescribing doctor to monitor your state of health and the good progress of your treatment.

How can one have access to this service?

Life I Hospital at Home service will be implemented at the request of your doctor or following a referral by the patient or next of kin.

The doctor and the nurse coordinator will proceed to a medical assessment at home for a potential admission in Life I Hospital at Home service. Your state of health and the care to be provided must meet predefined criterias derived from international recommendations

Your home must be suitable for your hospitalisation. A pre-home visit is thus required.

The team reserves the right to refuse hospitalisation if the criteria are not met.

If you are eligible for Life I Hospital at Home care, the multidisciplinary team will come to arrange your room and you will have the same services and the same equipment as in a healthcare facility.

The admission process

Step 1.

Your doctor directs you towards hospitalization at home or you/your loved ones discuss and agree with your doctor that your care and treatment will be continued within the framework of Life I Hospital at home services.

 

Step 2.

Life I Hospital at Home mobile team assesses the request and contacts you for an evaluation of your home place and health plan.

Step 3.

If the evaluation is positive, and subject to your acceptance of Life I Hospital at Home services, your personal information, your medical file, your discharge plan, and your inpatient insurance coverage (if applicable) are requested.

Step 4.

Your room is fitted out, if necessary, by the mobile team.

All the necessary equipment (oxygen, etc.) for your treatment is supplied and installed. Prescribed medications will also be provided.

Life | Hospital at Home Team takes over with regular home visits to provide care and may stay in contact with your primary doctor, if needed/requested.

Life I Hospital at Home care can only be carried out in an environment previously validated by the mobile team (selection criteria in terms of cleanliness and accessibility, presence of a sitter or a relative, understanding of instructions, etc.).

How to benefit from Hospital at Home care services ?

The request for hospitalization at home is generally initiated by the doctor but, in some instances, the patient or his relatives can make the request, after consulting the doctor.
